I've became the defender when I needed to be the attacker because we both would launch our attack against eachother on the same turn, and it's cost me massively more than once. What decides who attacks and who defends when two armies attack eachother, and what causes an army to evade attack from an army and launch their own against a city?

It depends who did his moves firstly. Who moves first, it will attack first too.

When you attack player city , units with more attack points attack 1st
when player attack your city , units with more defend points defend 1st
when 2 players attack the same country , that mean you once be the attacker and you once be the defender . depends .

As i said it depends on your move order. For example you attacked france in your first move and france attacked first london or etc and attacked germany as second move. In this situation you will attack and he will defend. Who make his move first, it will be attacker.
